When requesting for a replacement, you cannot check out any other items with this. Otherwise your replacement request will be canceled and you will be fully refunded. 
Please understand that the actual product is free, The price is for shipping and handling fee.
 
Before you complete check out, please COPY AND PASTE and fill out the 'Add note to seller' portion with the following information:
Name of the Device:
Invoice / Order Number: please specify where the item was purchased from.
Please make sure all of the information is correct, otherwise your replacement request will not be processed if any of the information is invalid.
 
This only allows you to request for (1) replacement requests per order so please make a separate order if more than 1 is needed or fill out our warranty form.
